1) LATE REGISTRATION ENTRIES:  Please select this form if you are registering an unattached individual late.  You can not use this for team entries.  When you select this form, you will have the option to open it to print a hard copy or save it to a file.

2) TEAM TENT RESERVATION:  Please select this form if you have registered 10 entries or more and you want space in the designated tent area.  Tents will not be allowed in the stadium unless they have been pre-approved using this form.

3) HOTEL REQUEST WORKSHEET:  Please select this form if you request BAYTAF to assist you with the discounted room rates.  When you select this form, you will have the option to open it to print a hard copy or save it to a file.

4) BAYTAF OFFICIAL:  Please select this form if you would like to work a BAYTAF Event.  Official's are the core to the success of any track and field meet, so we would like to thank you in advance for your support.

Note:  If your form requires a fee (no cash please), Cashiers Check or Money Order only must be sent with each form.
